Once you have completed the form in Step 1, please send it to Carrie Hoff at hoffcj@chipfalls.org. Carrie will then set up a meeting with you and the Director of Human Resources to discuss next steps. (May include medical certification)

After you have met with the Director of Human Resources you may need to give this document to your medical provider to complete.
