The Story of Cardinal Care Mental Health Supports:

In 2014 a need was identified to start a School Based Mental Health Program (SBMH) here in Chippewa Falls. SBMH refers to the delivery of mental health services in a school provided by an outside agency. The services are administered to students as needed as determined by a licensed therapist from an outside agency.

In the 2014-2015 school year, SBMH was started with one therapist from one agency. Our program has now grown to partner with multiple agencies and multiple therapists to help meet the needs of our students.

In 2017-2018 our district created a District and Community Leadership Team in order to brainstorm ways to bring our community therapists into our buildings more to work with our staff. This is called an Interconnected Systems Framework. The goal is to find ways to give our staff resources to use with students to help teach our students resiliency. This is where our mission statement came from. Our mission is that our community partnerships will promote staff awareness and increase understanding of mental health supports and resources so all students will develop life-long resiliency.

In 2018-2019 our district partnered with REALiving in order to offer a Student and Family Assistance Program. This is where each of our students or their family members can receive four free counseling sessions (four per family).

In 2019-2020 the CFAUSD was awarded the Project AWARE grant. This is a 5-year statewide grant that we share with the School District of La Crosse and the Lakeland Area School Consortium.

In 2021-2022 Group Health Cooperative of Eau Claire gave a generous donation to fund 4 free parent coaching sessions as part of our Student and Family Assistance Program. In 2022-2023 that expanded to also include youth coaching.

We also offer mental health resources within our school buildings such as screening, interventions, and access to school staff members.

FAQ

Our school district is able to fund 4 free sessions per family (students and/or immediate family members of the student) per school year. To get assistance in getting the 4 free sessions set up or to ask questions about SFAP, please call Shannon Larson, Mental Health Coordinator/Navigator, at 715-723-5542 ext 6405.

The process for setting up services includes selecting a community therapist who is an affiliate of REALiving, INC, provides School Based Mental Health (SBMH) therapy in the CFAUSD and has an opening. REALiving is available to be reached 24/7 at 855-838-7170. REALiving will email you and the therapy agency an authorization code that will be used to bill four sessions. The parent/guardian will reach out to the therapy agency to get the first appointment scheduled and complete paperwork.

No. The parent will need to call REALiving at 855-838-7170 to get the authorization code for the four free sessions.

Yes. They are able to call REALiving. However, they are not going to be able to schedule their own sessions in most cases, due to parents/guardians needing to consent to therapy.

If you know that you might continue with School Based Mental Health(SBMH) services after the 4 sessions, there are a few more things to be aware of.

  1. You will need to pick a therapist who is in-network for your insurance.
  2. Not all Student and Family Assistance Program (SFAP) therapists are available for SBMH. Some SFAP therapists are only available to be seen in the therapist’s office or don’t accept insurance as payment. You will need to select the therapist based on where you want your student to be seen.
  3. Any sessions delivered after the 4 free sessions will be billed to your insurance.

For assistance, or to ask questions, please call Shannon Larson, Mental Health Navigator, at 715-723-5542 ext 6405.

REALiving has a list of affiliate agencies and will provide you with that information when you call them at 855-838-7170.
